Structured Family Caregiving

Family Care, Inc. is an approved Structured Family Caregiving (SFC) provider agency serving Eastern Georgia. SFC is a Georgia Medicaid program that provides financial support and professional oversight to eligible family members who care for their loved ones at home.

Supporting Georgia Families Who Provide In-Home Care

Structured Family Caregiving enables qualified individuals enrolled in Georgia's CCSP or SOURCE waiver programs to receive care from a family member while that caregiver receives:

  • Financial compensation for providing care
  • Training and ongoing education
  • Support from registered nurses and care coordinators
  • Access to respite care services
  • Professional oversight and guidance

Important: This is a Medicaid-funded program with specific eligibility requirements. Not all families will qualify.


Structured Family Care

Eligibility Requirements

For the Person Receiving Care:

  • Must be enrolled in Georgia Medicaid
  • Must participate in CCSP or SOURCE waiver programs
  • Must require a nursing home level of care
  • Must choose to receive care in their home
  • Must live with the family caregiver

For the Family Caregiver:

  • Must live in the same home as the care recipient
  • Must be related by blood or marriage (examples: daughter, son, grandchild, daughter-in-law, son-in-law)
  • Cannot be: spouse, legal guardian, or parent of a minor child receiving care
  • Must be at least 18 years old
  • Cannot have any other employment
  • Must pass a criminal background check
  • Must complete required training (minimum 8 hours annually)

Important Information About Costs

  • No cost to the caregiver for participating in SFC
  • Care recipients may have cost-sharing obligations under their CCSP or SOURCE waiver program based on their income
  • These cost-shares are determined by Medicaid, not by Family Care, Inc.
  • We will clearly explain any potential costs during the eligibility process

SFC Eligibility Assessment

We help determine if you meet SFC requirements and are eligible for financial compensation. You may qualify if you live with the person as the primary caregiver, are related biologically (e.g., daughter, grandson) or through marriage (e.g., daughter-in-law, son-in-law), and the person you care for is Georgia Medicaid-eligible and participates in the CCSP or SOURCE waiver program. Note that spouses of waiver participants or legal guardians are not eligible for compensation.

Caregiver Training & Qualification

We ensure all family caregivers are properly prepared and qualified to provide care. This includes comprehensive initial training tailored to your loved one's specific needs, ongoing education, and verification of all program requirements.

Ongoing Caregiver Support

SFC offers payment to alleviate financial pressure and allow caregivers to focus solely on caregiving in the shared home environment. This service also provides tailored support to help caregivers meet participants' existing needs and plan for future changes in their needs and capabilities. Additionally, SFC offers backup caregiver support or respite care, which allows caregivers to attend to other essential tasks, including self-care, while ensuring the participant's safety.

  • Web-based tools for care documentation

Quality Oversight

Family Care, as an SFC agency, is responsible for ensuring that caregivers are qualified and trained to carry out their assigned duties effectively. We also guarantee that caregivers respond to changes in the participants' needs and complete their tasks as needed. As required by Georgia, we provide caregivers with web-based support tools for tracking information, such as daily notes that can be shared between the caregiver, care coordinator, and others. Additionally, we employ registered nurses who are available to answer caregivers' questions about their duties and who, along with each participant's care coordinator, visit the participant's home at least once a month to ensure that their needs are being met satisfactorily.
