At our Homeplace adult day center programs, we offer a variety of engaging activities, including community visitors, arts and crafts, Seniorcise, gardening, bingo, pet therapy, and seasonal and social events. We believe in providing a safe and welcoming environment where our patrons can participate in activities that promote socialization, creativity, and overall well-being. Our goal is to offer a daytime "home away from home" experience that helps individuals maintain their independence while receiving professional assistance with their healthcare needs.
The Homeplace adult day centers are designed to meet the needs of individuals who may require some level of assistance with their daily living activities, such as those with physical limitations, cognitive impairments, or chronic health conditions. Our team of professionals conducts a thorough assessment of each potential patron to ensure that we can provide the level of care and support needed. Additionally, we offer a trial visit so you and your loved one can experience the center and its activities firsthand. Our goal is to provide a safe and welcoming environment that promotes independence, socialization, and overall well-being.
Our Homeplace adult day centers have a staff-to-patron ratio that meets or exceeds state requirements. Our team includes certified nursing assistants, registered nurses, licensed practical nurses, and activity coordinators who work together to provide personalized care and support to each patron. This allows us to provide individual attention, monitor health status, and quickly respond to any needs or concerns that may arise. Yes, we encourage family members and loved ones to visit and participate in activities at our Homeplace adult day centers. We believe that family involvement is an essential part of our patrons' well-being and quality of life. Visiting hours are flexible, and we welcome family members to join in activities, meals, and special events. Our team also provides regular updates on our patrons' progress and is available to answer any questions or concerns you may have.
You may meet Structured Family Caregiving (SFC) requirements and be eligible for financial compensation if:
- You live with the person as the primary caregiver
- Are related biologically as a daughter, grandson, etc. or are related through marriage as a daughter-in-law, son-in-law, etc.
- The person you take care of is Georgia Medicaid-eligible and participates under the CCSP or SOURCE waiver program
Please note that spouses of waiver participants or legal guardians are not eligible for compensation. Our team can help you determine your eligibility and guide you through the application process.
Structured Family Caregiving (SFC) comprises three key elements:
- The caregiver-participant relationship - SFC is based on caregiving in a private home where both the caregiver and participant reside.
- Enabling caregivers to prioritize care - SFC offers financial compensation to alleviate financial pressure, allowing caregivers to focus on caregiving. It also provides tailored support to help meet current needs and plan for future changes, including backup caregiver support or respite care.
- Caregiver oversight - SFC caregivers are supported by agencies like Family Care that ensure proper qualification and training. We provide web-based support tools for tracking information and employ registered nurses who are available to answer questions and make monthly home visits along with care coordinators.
